Scarborough COVID-19 Vaccine Clinic: Pre-Registration for 80+

The Scarborough COVID-19 Vaccine Clinic website is now accepting pre-registration for residents of Scarborough who are 80 years of age or older. I encourage you to share this information widely with your family, friends, and network.
Residents of Scarborough who are 80 years or older interested in receiving more information and interested in pre-registering for the vaccine can use the following link: www.scarboroughcovidvaccineclinic.ca
Please be aware:
- If you do not live in Scarborough, or are not 80+, you will not be eligible to receive the vaccine. Please do not continue with this form. See pre-registration forms for other regions below.
- You cannot schedule a vaccination appointment with this form. Filling it out allows you to pre-register so that the clinic can email you directly to book an appointment when distribution of vaccines for those age 80+ in Scarborough is set to begin.
- You will not receive information about booking your appointment until you are eligible.
Please ensure you consult your primary care provider before booking an appointment. Also, do not schedule your appointment if you have symptoms of COVID-19 or have answered 'yes' to any questions on the COVID-19 Screening.
Immediate priority for first-dose vaccinations includes:
- Staff and essential caregivers in long-term care homes, high-risk retirement homes and First Nations elder care homes, and any residents of these settings that have not yet received a first dose of vaccine.
- Alternative level of care patients in hospitals who have a confirmed admission to a long-term care home, retirement home or other congregate care home for seniors.
- Highest Priority health care workers, followed by Very High Priority health care workers, in accordance with the Ministry of Health's guidance on Health Care Worker Prioritization.
- Homeless shelters.
- Indigenous adults in northern remote and higher risk communities (including on-reserve and urban communities).
When all reasonable steps have been taken to complete first-dose vaccinations of all staff, essential caregivers and residents of long-term care homes, high-risk retirement homes and First Nations elder care homes, first-dose vaccinations may be made available to Phase One populations):
- Adults 80 years of age and older.
- Staff, residents and caregivers in retirement homes and other congregate care settings for seniors (e.g., assisted living).
- Health care workers in the High Priority level, and in accordance with the Ministry of Health's guidance on Health Care Worker Prioritization.
- All Indigenous adults.
- Adult recipients of chronic home care
